Referral form

The patient referral form on our home page is for display only. Nothing you enter is sent to us or saved. To complete a referral, please call our office.

Employment application

When you choose to email your application, the website opens a new message in your own email program with your answers filled in. The information is sent only if you press send, and it travels through your email provider to our office. If you print the application instead, nothing leaves your device.

Applications we receive are used only to evaluate candidates for employment, are kept confidential, and are shared only with staff involved in hiring.

Information you give us directly

When you call, email, fax, or visit our office, we use the information you provide only to answer your question, arrange services, or process a referral or application.

Health information

Health information about our clients is protected under federal and state law, including HIPAA. This website does not collect health records. Clients receive our separate Notice of Privacy Practices, which describes how medical information about them may be used and disclosed. Please ask our office for a copy.

Third party services

This website does not load fonts, scripts, or other content from outside companies. Like most websites, our hosting provider keeps standard technical logs, such as IP address and browser type, to keep the site secure and running. Links to outside websites are provided for convenience, and we are not responsible for their privacy practices.

Children

This website is not directed to children under 13, and we do not knowingly collect information from them.
